The Forms and Information that we MUST have signed and returned to the office before school starts on Tuesday, September 5th are the following:
  • Enrollment Agreement
  • Health Form
  • Updated (with in one year) Physical and Immunization Records faxed to (508)632-6555 or emailed to office@laurel-school.org or amy@laurel-school.org
  • Lead Screening - All children in our Toddler, Preschool & Pre-Kindergarten Programs MUST send this in as well!
  • Elementary Students in Grades 1-5 will have all their supplies and Field Trips etc. covered by the Student and Activity Fee
