The Forms and Information that we MUST have submitted on Remini before school starts on Tuesday, September 3rd are the following:

  • Health Form
  • Updated (with in one year) Physical and Immunization Records 
  • Lead Screening - All children in our Toddler, Preschool & Pre-Kindergarten Programs MUST send this in as well!
